Sports Physical Screenings

Defiance Clinic ProRehab provides the Ohio High School Athletic Association required pre-participation sports physical for athletes entering grades 7-12.  Our staff annually provides over 350 sports physicals on-site at Ayersville, Tinora and Defiance high schools.  On-site physicals include: vital sign checks, a musculoskeletal exam, and a general health exam.  The cost of physicals at the school is $20.00.  If your school is not listed, or you can’t make the date set for your school, you can get the required physical by making an appointment with one of our Family Practice Physicians.  Below are the dates for sports physicals at the respective schools.  Please see your athletic director for the appropriate form necessary for the physical.

2007 Sports Physicals

April 30, 2008              Tinora

  May 7, 2008            Ayersville

      May 14, 2008            Fairview

    May 28, 2008            Defiance

Nurses should be at school by 1:30pm

Doctors should be at school by 2:00pm
