  Defiance Clinic ProRehab provides the Ohio High School Athletic Association required pre-participation sports physical for athletes entering grades 7-12. Our staff annually provides over 350 sports physicals on-site at Ayersville, Fairview, Tinora and Defiance high schools. On-site physicals include: vital sign checks, a musculoskeletal exam, and a general health exam. The cost of physicals at the school is $20.00. If your school is not listed, or you can’t make the date set for your school, you can get the required physical by making an appointment with one of our Family Practice Physicians.
