Bring Your Insurance Cards to Every Appointment
Please present your current insurance card or cards at Central Check-in when you arrive for your appointment. Our staff will verify that we have correct information for you.

Co-pays
If you have an insurance co-payment, it will be collected at Central Check-in.

Automatic Filing
You can arrange with the staff at Central Check-in for automatic filing of primary and secondary insurance.

Defiance Clinic is a provider for Buckeye Traditional Medicaid only. If you would like to sign up for Buckeye or switch from another plan to Buckeye, please call 1-800-605-3040 for more information. That is the toll-free number for the Ohio Selection Services Center for Medicaid managed care plans.

Disability Forms
Please complete your section of the form and bring it to the Insurance Office. The staff will see that the physician’s part is completed.
